 我的人生太順利了。35 歲就當上主治兼外科加護病房主任,台大一百年來找不到第二個。以世俗的眼光來看,我好像什麼都有了,成就、名利、妻賢、子孝。
 但我不快樂,連家都不想回
 這輩子我從沒做過自己想做的事。我念台大醫學系,不是因為想當醫生,是爸爸幫我填的志願;結婚是我媽替我相親;至於要生幾個孩子,我太太做的主。但我問自己到底想做什麼,卻想不出來。真可笑,一個 45 歲的男人,還在領壓歲錢。
 我沒有養過父母,爸爸比我還會賺錢。到台大上班的第一天,他對我說:「 工作不要失去人格,放手去做,反正你的退休金我都準備好了。
 我確實很拼。年輕時還有救人的熱情,曾經是地獄不空,誓不成佛;後來發現地獄實在太大,救不完。巡一趟病房,30 秒內要決定病人的生死,情感就成了奢侈品。現在我對人完全無感,人的心在想什麼,我不知道、不想知道、也不用知道
 10年來,我花太多時間在工作上,突然渴望家人的擁抱時,家已經不是個家了。我兒子三歲前沒看過我,因為我回到家都在睡覺,太太指著我跟兒子說:「這是爸爸」。後來小孩還以為爸爸就是睡覺的意思。我太太勤儉持家,但我們很少說話,孩子是她的全部,我總覺得我在家是多餘的

 惡性循環吧,我更不想回家了。每天在醫院超過 14 小時,撐不住才回家洗澡睡覺,有時還故意不回家。在小小的辦公室裡,我寂寞到發瘋,甚至想乾脆出家好了。
 最可悲的是,我跟老爸說我想出家,他竟回我:「那我蓋一座廟給你」。不依循別人為我設計好的模式而活,看來我是永遠都甭想了。還是回家吧,可是回家的路好漫長啊。
 【後記】這是7年前在壹週刊的文章,也是我在45歲時對生命的迷惘。埋頭在工作上耕耘了10年,當在葉克膜及外科重症照護耕耘方面有了初步的成績時,回頭省思生命的意義及價值時,卻產生了極度的困惑與懷疑。在當時的迷惘後,這幾年又逐漸的對生命有了更深層的體悟。

有 一天,黃勝堅醫師煞有介事的對我說:「我們外科加護病房必須注重安寧照護!」。初次聽到,當然不以為意。事實上,我和黃醫師都是外科重症頂尖的專家。黃醫師專精於神經重症,沒有頭的病人(腦死病人),一般最多撐不過兩個星期,他卻有能力維持數月之久;我是心肺重症專家,沒有心臟的病人,使用葉克膜(ECMO),也可維持十六天,再接受心臟移植,最後病人清醒的自己走路出院。
台大外科加護病房,在我們聯手打造之下,早已是世界級的重症醫學中心,怎會到有一天(20047月),我以行政命令宣示:「安寧照護是外科加護病房的工作重點,有關的臨床服務、研究發展皆列為優先項目」

我自從專職外科加護病房工作以後,承蒙當年的上司朱樹勳教授大力支持,外科重症是整個台大外科的重點。人力、物力之支援皆是第一優先,因此器官移植、葉克 膜、人工肝臟、各種透析技術、各種人工維生系統,不過幾年之光景,就追上世界水準。曾有一段時間,台大醫院的記者招待會,和我們外科加護病房有關的就占一 半之多,當時真覺得「人定勝天,科技萬能」,心中好不得意。
衣帶漸寬終不悔,為伊消得人憔悴
無奈科技終究有其極限,胡夫人邵曉鈴、星星王子、……固然是令人欣喜的成功案例,但也有不少救不活、卻也死不去的,甚至可說是「灌流良好的屍體」。面對焦慮的家屬,狐疑的同事,甚至自己站在病人的床邊,挫折的無奈竟然掩蓋了所有過去的欣喜,變成揮之不去的夢魘。

慢慢的,終於了解人生有「生老病死」,就如氣候有「春夏秋冬」。「天何言哉?四時行焉,百物作焉」,終於領悟醫師就是醫師,其目的只是替人世減少苦痛,不管是身體的或精神的。人生花園之中,醫師只不過是一名園丁吧!我們不能改變「春夏秋冬」的循環運行,卻可盡力讓人生的花朵更加燦爛。有時雖是園丁照顧花草,有時反而是花草的枯榮在渡化園丁。 
一段往事
曾有一位大老闆,在事業正盛時,罹患克雷氏桿菌肝膿瘍。開刀引流後,卻引發嚴重敗血症併發急性呼吸窘迫症,最後被迫使用葉克膜維持生命。病況最嚴重時,呼吸器每次通氣量不到100㏄,後來更併發急性腎衰竭,在葉克膜之管路上再架設洗腎的管路。當年正好國際外科醫學會在台北舉行,葉克膜的祖師爺巴特雷醫師Dr. Bartlett也受邀來台與會演講;順道拜訪台大醫院時,帶他參觀加護病房,結果他在此病人床邊站了一個小時,東看西看直說:「Wonderful」。後來他到處跟人家說,台大的葉克膜是世界最強的團隊之一。

經過55天的漫長葉克膜治療,終於把病人搶救回來。對醫療團隊而言,與其說是高興不如說是得意。後來轉到普通病房後,突然有一天病人有急性盲腸炎,當時只想真是禍不單行,不過還是立刻安排緊急手術。術後開刀醫師告訴我,闌尾看起來發炎不嚴重,倒是盲腸壁感覺較厚,開完刀後一切順利。出院後不到半年,在一次例行胸部X光片檢查發現有一顆腫瘤,細針穿刺檢查之病理報告赫然是淋巴瘤,電腦斷層發現腫瘤已沿著主動脈蔓延到整個中膈腔。至此回想,才知道原來一開始是腸胃道淋巴瘤,造成腸黏膜潰瘍,細菌藉此侵入引發細菌性肝膿瘍以及後續的一連串事件,後來的急性闌尾炎,只是局部的併發症而已。
知道真相後,原有葉克膜治療成功的喜悅一下子被澆息,當然也替病人找了最好的醫師、用上最好的藥物。初期的治療效果不錯,但腫瘤卻一再復發,最後望著胸部X光片,看著腫瘤一天一天的變大,變成我最大的痛苦。
害怕病人問我:「有無其他治療方法?」
也痛恨自己含糊回答:「我再想想。」
事實是已無法再想了。
有一天,病人突然對我說:「我這一關死定了。我很謝謝你的努力,你就不要再有壓力了」,我們兩人無言相望半响。後來我通常是忙完一天的事,晚上十一點多才去看這個病人,通常家屬也回家了,空盪的單人病房變成醫師和病人的午夜會談。
這麼多年過去了,治療過程的欣喜、挫折,都忘記了。唯一還有記憶的,卻是兩人午夜聊天,甚至是兩人的相對無言。最後這一段日子,因兩人的互信互諒,我們做到了生死兩相安,再無遺憾。他,走的很平靜。從此我知道醫生在診斷、開刀、藥物治療以外,還有一些可做的事,甚至什麼事都沒作的相對無言之中,也有醫師的價值在其中。

大四剛當見習醫師時,初次穿上醫師袍。要去看病人之前,都會先問護士姐姐,打聽一下病人來自哪裡?作什麼工作?有那些主要親屬?那時候,看到的每位病人都是一個完整的病人,有七情六慾,是家中的一員,是社會中的一分子。我不但看到病人,也看到床邊的家屬。
後來醫術日益精進,擠身名醫之列,看到轉診紀錄,瞄一眼抽血數據,系列心電圖逐張看過去,床上的病人都沒有看到,已脫口而出:「急性心肌炎」。有好幾年的時間,我只看到「器官」,沒看到「人」;只看到「病」,沒看到「病人」;更不用說是旁邊的家屬。
直到最近才又重新看到「病人」了。「病人」不再只是數據、超音波、病理報告的組合;而是一個有喜怒哀樂,在家庭、在社會中牽扯不清的一個人。
黃勝堅醫師近幾年,誓言要做「生命導航者」,要在生死迷惑之間,引導眾生走過困惑。我笑言:「你連自己都迷路了,還當別人的嚮導?」黃醫師卻正言說:「在一片迷惘之中,至少我一定陪伴他們一起走到最後一刻」。
在「生死謎藏」一書中,黃勝堅醫師述說著三十六個生死之間的故事,希望大家讀了這些故事,儘管「春夏秋冬」仍然不停運轉,但人生的花朵皆能更加燦爛光輝。

蜂蜜是由蜜蜂採集花蜜,經過儲存、釀製、熟化後採出的食品,蜜蜂採集花蜜,按植物的不同,可分為龍眼蜜、荔枝蜜、柑桔蜜、向日葵蜜等,冬天由於植物花期不長,蜜蜂採集多種植物的花蜜混合通稱為冬蜜

另一種將蜂群餵糖採收的蜂蜜,通稱為糖蜜,是不純正、不自然的產品。

蜂蜜含有50%~80%的果糖、葡萄糖,不經消化可直接被身體吸收,含有蔗糖、多糖、蛋白質、礦物質鐵、鉀、鈣、鎂、銅、錳、磷等,有機酸、活性酶、芳香物質、維生素等營養物質。

以同重量糖類蜂蜜所含熱量較黑糖、白糖、冰糖為低,是甜度低熱量的好食品。蜂蜜的品質優劣及成分受蜜源植物種類、採蜜期氣候、養蜂者的管理、及採蜜方式等的影響很大,因此需鑑別蜂蜜的真偽好壞。

蜂蜜性味甘、平、功可補中益氣、潤肺止咳、潤燥、止痛、解讀、矯位、具有補中緩痛、解毒潤燥的雙向性功能,許多中藥物的炮製及丸類藥物的製作,都需蜂蜜的加入達到功效。以蜜茶來說蜂蜜一大匙,水一杯200C.C,長期服用對便血、便秘、咽痛、慢性支氣管炎、高血壓、心臟病都是不錯的飲品。

禁忌方面』
蜂蜜很容易受細菌的汙染,例如黴菌及酵母菌或肉毒桿菌等,五歲以下的幼兒由於胃腸功能尚未發展成熟,較容易致病,成人抵抗力佳,服用蜂蜜則不會有問題,另外具有過敏體質者也應先試用,因為蜂蜜中具有人體與不同的酵素,對氣喘、蕁麻疹、異位性皮膚炎、過敏性鼻炎可能會引發過敏反應,有過敏體質者,試用若無任何反應即可安心食用。

In the class of 1963, we have approximately 75 friends (among 146 graduates) who have made known their e-mail addresses.  During the past few months, I have been trying to contact most of them. So far, we have only 50+ in my contact list. It means only one third of them are among frequently contacted.

Every day I have been sending “Hello” messages with attached articles, among which there are some forwarded ones, Power Points, and some of my own writing.  I am sure that in general you guys show interest in reading them, especially my own writing.  However, a saying goes, “He who tries to please everybody pleases nobody.” I apologize for not completely complying with personal interests of some at all.

Since there have been few feedbacks received, I do have a hard time understanding the key interest of yours. Certain difficulty arises as to how I should emphasize main ‘likes or hobbies’ of classmates to please you all.

Currently, there are about 15 people who declined to receive articles sent to them, and I, per their request, already deleted their names from contact list. There are some who gave a flat refusal to receive my articles, some with a white lie (lousily told) as excuse, and still others simply with remarks, showing anger at my articles.

Well, it’s been my pleasure to service all of you. However, from time to time, a notice of refusal to continue receiving my articles from someone is just like a dagger stabbing into my heart. Yes, you are not obligated to keep receiving them. But when expressing your request to discontinue receiving articles, please at least show your politeness, decency, ethics, or good manners.  To us elderly people, an unpolished or rude “request” is indeed hurting someone’s feelings. 

Those bluntly doing so, seem to me, to declare a solemn ‘termination’ of friendship (斷交) . I will comply with such a request of course, but I deeply regret the necessity of such a drastic measure taken by them. After over 50 years, we guys came to be happily reconnected with each other. But do we have to suddenly ‘bid farewell’ simply because we do not concur with each other’s points of view?  Please be so kind as to let me know what you think.

古老聖經的台灣預言?

「你的財寶在哪裡,你的心也在哪裡」,這句出自新約聖經馬太福音的智慧箴言,本是提醒基督徒不要為了物質生活,而忘了精神信仰上的追求,但不知為何,看著滿城風雨的阿帕契風波,我心中卻再度浮現了這句話。若說一個人的財寶在哪裡,他的心也在哪裡,那麼一個社會的價值觀在哪裡、富人階級把財寶放在哪裡,是否也預示了這個國家的未來在哪裡呢?

一位女星意外上傳的阿帕契軍機照,竟成為台灣可說是有史以來最嚴重的軍紀案。對此,我相信所有台灣人都毫不意外,卻又依然深感荒謬。軍紀潰散、軍中特權事件頻傳,權勢凌駕法治之上,這早已不是新聞,更不須女星特意揭穿瘡疤。但洪仲秋虐死案才發生一年多,才剛有50萬人走上街頭要求軍隊改革,軍方與國家高層卻依然不痛不癢,官官相護,以各種藉口挑戰人民智商,這是荒謬;不去檢討這個國家從上至下有多少特權制度(光是這一年來就有數不清的政治酬庸,如郭冠英省政府破格任用等爭議),只連帶拿幾個小兵開刀,虛晃一招改革之「決心」(真有決心,早在50萬人上街頭加上一條人命的代價後,就會大刀闊斧了!現在看來,只是事到如今不得不止血的敷衍心態),這是荒謬;女星第一時間一副輕佻沒事樣,後來才見局勢不對,連忙對貴婦團友人們為自己的「不當PO文」道歉,聲明背後的潛台詞更是荒謬,似乎暗示著:「都是我上傳了照片,才害國軍藏好好的形象ㄅㄧㄚˋㄎㄤ(破空)──原來特權是應該,錯只錯在太高調,忘了要偷偷來;為女星聲援的藝人友人、豪門貴婦指責這整件事是包裝著公平正義的仇富心態,但事實真是「仇富」這麼簡單嗎?

「仇富說」讓我想起了金門的「金水國小」,有著金門獨特的閩式洋樓建築風格,別緻又大器,原是20世紀初華僑在南洋賺了大錢後,為了回饋鄉里而投資的教育事業。類似的學校全金門還有56所。這筆投資並不太可能讓經商致富的僑商們得到更多金錢上的收益回報,卻是當時「富人」的必要責任。相較之下,實在很難在現今台灣富豪身上看到同等心態,多的是深怕和平民念同個學區,降低了明星光環之封閉心結,還是固守小圈圈,讓子女們在學校結交其他名門之後,自小「培養關係」比較重要。當然,這可不是專屬於20世紀初金門人的獨特氣節。世界上多的是海外致富的商人,將大筆財富投資於母國社會,像是以色列建國、希臘獨立運動,甚至是中華民國辛亥革命(如從小國民教育所灌輸的歷史背景),都是憑著富庶僑民的財力支持,才得以實現。

的確,並非所有商人都有著如此高尚、愛鄉愛民的情操。想要炫耀自己身分地位、與人一較高下,自也是人之常情。古今中外,「炫富」的方式有千百種。既然要炫耀,自然是能流傳越久越好,讓越多人看見越好。於是,在雄偉壯觀(有時甚至過分華麗)的教堂、寺廟、神殿之外,「有機會永垂不朽」的藝術作品,無論是詩歌、音樂、繪畫,都成了另一種「炫富」之投資。話說回來,也得要感謝這些前人的炫富心態,讓過去活在幾百年前的藝術家們,能找到些「運氣」匹配自己的才華(有時適度的諂媚討好也是不可少的,端看藝術家與資助者的關係而定──「藝術」從來就不是絕對清高的),也讓後人如我們能享受到這些作品的陪伴。就算富人們只是附庸風雅,但這些同樣「無以回收」的投資(就金錢面而言,而藝術品要增值往往也是好幾代之後的事了),對他們來說依然是值得「炫耀」的。即使到了近代,民主社會對於公義平等的要求更高了,富人階級自然也無法像在過去貴族社會,得以踩著農民、平民、奴民,擁有無限累積的財富,但資助藝術文化創作的傳統,在許多歐美國家都未曾斷過。無論他們的出發點是為了私利或公益,至少都成了藝術文化不可或缺的助力。

再回頭看看台灣,現今社會的富人階級似乎總愛把財寶藏在別處,用另種方法「炫富」。很可惜,他們的投資算盤可沒能讓我們小老百姓雨露均霑。名牌、派對、天價紅酒,這本是個人所選的生活型態,外人也無從置喙,至少還能促進頂級奢侈品的消費與生產。但竟然連「房產」這種本應是最基本的「居住權」,都成為炫富的物品、投資的工具。給人住的房子,竟成為生財法寶,真是荒謬至極。金門僑商蓋的洋樓,雖非人人住得起,至少賞心悅目,有時幾棟高樓還肩負起鄰里守望的治安大任,多年後更成為文化古蹟,與社會共享之資產;今日一批一批建案完工,多得是被投資客下訂後只買不住,市井小民買不起也沒得住,到夜晚如鬼屋般僅有零星燈火,完工兩年比十年廢棄老屋(醜的那一種)還汙濁。這樣的「富人」心態,讓我們的居住權、藍天綠地享受權、美觀豪宅觀看權一一被剝奪,如何期待社會大眾能把這些富人階級看做「於社會有益」的存在?

更荒謬的是,對於這些富人來說,「特權」竟也成了一種「炫富」途徑。阿帕契派對正是如此,炫耀的不只是軍機,還有「關係」。更有甚者,認為「當我真心渴望追求財富地位,整個國家都該聯合起來幫我完成」。美河市投資客不正是如此心態?房產投資本是有漲有跌(更別提這幾年漲幅有多不合理),政府連「居住權」都無從保障起了,竟還要幫投資客保本,這又是何道理?記得多年前,家母曾建議風險較小的基金理財,我開玩笑問她:「要是賠了你會幫我保本嗎?」媽媽只明智地說了一句:「你別作夢了吧!」如今投資客意圖耍特權要政府保本的心態,豈不值得這句回應?這樣的投資心態,連點膽量都沒有,更遠比不上當年那些賭上身家、遠走他鄉的僑商。要是在這塊土地上的富人,充斥了這樣的人格特質,還有什麼資格埋怨被敵視?引起社會反感的,又怎會是「財富」?還不如說,是這種只會把整個社會拖累的寄生蟲心態吧。

從人類歷史看來,絕對的公義早已被證明不存在(但不代表不值得去追求),人人均富的共產制度,看來也只有在人數不多的小部落有實現的可能。既然這世界注定要有富人、有平民,也許我們最微薄的期待,是少一點耍特權、任我行、凡事只看見自己的豪門之家,多一點願意投資文化藝術、願意關懷社會、甚或是如王爾德般能為後代留下一些美好事物的富二代!「你的財寶在哪裡,你的心也在哪裡」,人性如此,國家社會也如此。與其將財寶累積在錢滾錢的貪婪想望上,倒不如好好投資當下所處的這個社會,這才是真正有價值的「炫富」
