在台灣抗生素的誤用和濫用,約開始於1960年代。最多使用的藥劑以盤尼西林最多,但常因過敏導致死亡病例增多,很多醫生改用其他藥,如Tetracycline, Kanamycin, Erythromycin (紅黴素-俗稱Ilosone-對喉痛發炎有奇效) ,隨後Lincocin盛行,它對支氣管炎有效。後來1970-80很多醫生改用Keflex,到了1990年代,出現Amoxilin等。當時美國新抗生素一出來,台灣立刻可購得(即使無醫生處方),但在美國買不到。原因不在台灣較美國人先進,而是美國人『怕死』。因新藥的副作用除破壞腸胃外,其他未知的大小風險一概由先用者去承擔。一旦嚴重意外(malpractice)發生,台灣人不懂打醫療官司,現在也不懂,更無此風氣…...

The idea of the commons is deeply woven through the history of the English countryside. Shakespeare captured this idyllic approach to nature’s wealth in “As You Like It,” when the shepherd Corin explains to the cynic Touchstone the joys of his life. “I earn that I eat, get that I wear,” he says, adding that “the greatest of my pride is to see my ewes graze and my lambs suck” — in the unowned, readily shared Forest of Arden.

There can be trouble in such an Eden, as Hardin pointed out in an influential 1968 paper. Hardin asked what would happen if access to a commons were truly unfettered — if Corin and every other villager ran as many sheep as they could there. In such cases, Hardin argued, the endgame is obvious: Too many animals would eat too much fodder, leaving the ground bare, unable to support any livestock at all.

The evolution of resistance to antibiotics fits that story perfectly. The first modern bacteria-killing drug, penicillin, came into widespread use in 1944, as American laboratories raced to produce millions of doses in time for D-Day. The next year, its discoverer, Alexander Fleming, used his Nobel Prize lecture to describe precisely how this wonder drug could lose its power, telling the sad tale of a man who came down with a strep infection. In his tale, Mr. X didn’t finish his course of penicillin, and his surviving microbes, now “educated” (Fleming’s term), infected his wife. When her course of penicillin failed to eradicate these now-resistant microbes, Mrs. X died — killed, Fleming said, by her husband’s carelessness. It took just one more year for this fable to turn into fact: In 1946, four American soldiers came down with drug-resistant gonorrhea, the first such resistance on record.

Since then, human decisions, most importantly the overuse of antibiotics in agriculture and over- and mis-prescription of the drugs for human patients, have created the future that Fleming feared.  Multi-drug-resistant tuberculosis is a global scourge, and the first recorded deaths from TB untouchable by all known agents occurred in 2003. In 2015, E. coli displaying a novel resistance mechanism to the antibiotic colistin showed up in pigs, in raw meat, and in a handful of human patients in China — and colistin-resistant microbes have since spread to countries around the globe, including the United States.

This is potentially disastrous. Colistin is a drug of last resort for several classes of infections. In the language of the commons, we have drastically overgrazed our communal field of antibiotics, and humankind will pay the price. A team of experts convened by the British government reported last May that the evolution of resistance in microbes kills at least 700,000 people worldwide each year. Unremedied, that toll will worsen. By 2050, the same study warns, the loss could balloon to 10 million a year — more than the expected death toll from cancer.

ANTIBIOTICS ARE not the only modern-day commons at risk. The atmosphere is another. It can be disrupted at the local level — just ask the masked pedestrians making their way through literally deadly air pollution in any number of megacities. Globally, the cumulative impact of every drive to work, each cooking fire, and every gallon of oil burned to heat a home can be read in the daily measurements of atmospheric carbon dioxide at the observatory on the flanks of the Mauna Loa volcano in Hawaii: 408.28 parts per million as I write this, up from 404.2 a year ago (and 315 as first observed in 1958).

While the full consequences of this use — or abuse — of a truly global commons will only emerge over time, new risks emerge with ever passing year: increasingly damaging storms, droughts, and floods. Yet more fundamental shifts in the earth’s climate system are in sight, too. Potential changes in the circulation of the ocean or atmosphere would radically alter the weather patterns on which human societies have come to depend.

由三位美國東部小小大學的三位教授共同獲得。
Rosbash, Hall, and Young were each awarded a one-third share of the $1.1 million prize..三人獲得諾貝爾醫學獎,均分1佰10萬元獎金。Brandeis professors, once ‘mocked’ for research, win Nobel Prize for Medicine他們的研究曾經受到嘲諷。Brandeis, an institution founded after World War II. With nearly 6,000 undergraduate and graduate students today.洛克斐勒大學歷史很短,二次大戰後創立,僅有大學生、研究生约6,000人。今天這所小學校大出風頭。

曾經有位日本著名醫學專家曾說,許多病人每日服用的醫藥(medication) ,多半並不需要,亦即不服用也不影響病情。真是這樣也許是吧。尤其是疼痛等慢性疾病(chronic diseases) ,如關節炎(arthritis) 或與代謝作用(metabolism)相關的病痛。依本文作者看法,長期服用止痛藥(pain-killer)如習慣成性(habit-forming) ,對心理會造成影響,因為病人基於某種信念,認為服了藥就會止痛,對醫藥的依賴也隨之加深。這就是作者認為「思想能影響人體基因的道理。

To clarify, “influence your genetics” means to turn genes on or off. Not all genes are accessible to modification. We have critical periods in development during which certain genes may be influenced before they go silent. For example, the genes that build the structure of your body. We have only one window of time during which to build arms, legs, teeth, etc. During this window, things like drugs, maternal illness and nutrition can affect genetic expression. Once that window closes, we cannot “will” to grow a new tooth or replace an amputated limb.

Other genes are operative throughout your life, making hormones and enzymes, building proteins to digest food, replacing cells, repairing damage, etc. . These are the ones which might be influenced.

The simplest demonstration that we do influence our genes by our thoughts is the Placebo effect, in which an inert substance can behave like a real drug.

When we are given a Placebo pain killer, the mere belief in the power of this pill causes us to generate real endogenous molecules which actually do block pain.

This can be proven by giving Naloxone to a person after he begins showing a placebo effect of pain relief. Naloxone blocks the action of opioids (pain killers). Once you administer the Naloxone to the person experiencing placebo pain relief, the placebo effect disappears. That is, the pain returns.

This means the person had created their own opioids(from the mere belief that he would be relieved) and that these opioids (which effectively silenced pain centers) could be blocked (effect removed) by Naloxone, just as real molecules of heroin would be blocked.

Other examples are the calming effect and reduction of lactic acid brought about by meditation (Wallace RK, Benson H. The physiology of meditation. 1972). Or the therapeutic effects of Cognitive Behavioral Therapy in changing one's maladaptive (stress causing) thoughts into more productive interpretations. Changes in perceived stress levels have a dramatic effect on cortisol production (see Robert Sapolsky Why Zebras Don't Get Ulcers). It is rumored that prolonged exposure to stress can turn one’s hair grey and contribute to aging. Too Much Stress May Give Genes Gray Hair.

Studies of single mothers raising disabled children showed a shortening of telomeres at a rate thirteen times that of age matched controls (Epel et al . 2004) Happily, in providing these mothers with a support group, the production of telomerase, the enzyme which acts to restore some of this damage, will increase. So, ones’ social predicament influences one's biology.

In sum, within certain parameters, the mind and body operate dynamically. But you cannot wish yourself to be taller, slimmer or improve your features this way.

台灣仍有些生活在較落後地區的人們,迄今仍相信廟宇的神明如乩童能治病,乩童開列治病之藥方,僅是「香灰」加一張冥錢,燒化充水服用。奇怪的是,有時疼痛症狀消失。因為患者對所謂的「神明」和「乩童」具有確能治病的『信心(confidence and trust) 。有時當你感到疼痛異常,但到了診所,病情突然減輕三分之一,看到醫生再減輕三分之一,也許還未打針、服藥,完全不痛了。這是心理隨『生理基因』的轉變而形成的效應。因此有人說,有時服用自己信賴醫生開出的處方藥,藥效會相對增加,你相信嗎?
